The Graduation Transitions Program replaced CAPP and Graduation Portfolio as a graduation requirement for all BC students in 2008. GT mentors are This e-mail address is being protected from spambots.       GT worksheets are provided as a guided option for evidence of the requirements and are available in the Career Centre or on this website.

 

 

1. Healthy Living:

Daily Physical Activity

  • It is expected that you are keeping up your weekly/monthly log either on paper or online and will do so up to your exit interview in grade 12.
  • The Ministry of Education online tracker tool at www.learnnowbc.ca (you will need your PEN number)
  • LSS Graduation Transitions Daily Physical Activity Record (also available in hard copy in the Career Centre).  One record sheet per year in grades 10-12.

3. Graduation Transition Planning

Graduation Transition Planning  is a grad requirement and also an important exercise when planning your grade 11/12 courses and beginning your research into Post-Secondary education and training. 

The Graduation Transition Plan consists of:

  •  Career Option Research using Career Cruising or another online Career Research tool of your choice.
  • Personal Planning for a job/career of interest
  • Resume Writing
  • Completing an Application Form
  • Completing some Personal Financial Planning
  • Demonstrating Attributes of a BC Graduate

 

  

What Happens Next Year?

  • Graduation Transitions is a graduation requirement that will be listed on your Grade 12 Course Planning Sheet.
  • Students with 88 or more potential credits may opt to have GT in their timetable and use this primarily as study time while meeting the requirements of weekly tutorials.
  • You are expected to update aspects of evidence from grade 10/11 so that it is accurate for you as a graduate. You may file or save hard copies of your work from Planning 10 or Grade 11 Tutorials - either keep in a safe place of your own or in the file cabinet in the Career Centre.  You can save your work today to the S Drive for future reference and modification for grade 12.  
  • Creating and updating the required evidence is the responsibility of the student - you are treated like the independent adult that you are transitioning to.
  • You must do an exit interview on a designated date in the semester in which you have GT.
  • At the interview you must present evidence for all of the above requirements.
